Once your appointment is scheduled, you will receive two emails linked to intake forms. Out of respect for your time and to ensure a smooth first appointment, please complete the online forms prior to your arrival. Please do not hesitate to contact us at 913-732-3877 if you have any questions!
If you are unable to keep an appointment, as a courtesy to other patients and our staff, please give us 24 hours' notice. We reserve the right to apply a $25 charge to your account for each cancellation received less than 24 hours in advance. We also reserve the right to apply an $85 charge to your account for each appointment missed when we do not receive prior notice (no call, no show). The patient will be responsible for payment regardless of future appointment schedule.
